Going to a urologist & oncologist-always get a 3rd opinion

In 2018 I was 66yrs. Old, my urologist in Denver diagnosed me with prostate cancer with a Gleason score of 3+4, after only offering results of a biopsy and a PSA of 3.2,( this is a doctor I’ve known for 30 years or longer). He recommended I have my prostate out,(he told me that’s what he would do) I asked what are my options, he said we go see the oncologist down the hall(this is a very large urology practice in Denver Colorado), who recommended radiation. These were my only two options offered to me .
My MDVIP DR. Trojanovich recommended a 3rd opinion of seeing an internal medicine doctor in California, that is also a cancer oncologist specializing only in prostate cancer, he is not a surgeon.
He said the first thing I need to do is have a prostate MRI, which I did in Denver and the imaging facility indicated the prostate cancer was getting worse, Please note that this imaging facility has no protocol for prepping for a MRI of the prostate.
Prostate Oncology in California said please send us the MRI and will have our team at UCLA read it, they came back and said the cancer is NOT getting worse and the radiologist in Denver is reading inflammation.
Prostate imaging is complicated and imaging facilities are reading all types of imaging, what I have found, that to be proficient in Prostate imaging you need to read approximately 1000 Prostate images.
Since 2018 I now fly to Los Angeles and have my MRI done at UCLA imaging in Santa Monica every six months and then visit my doctor at Prostate Oncology in Marina del Rey just 20 minutes away, UCLA has strict protocols for imaging the prostate, they ask for no exercise three days before, a light diet, fasting the day before the MRI, and giving yourself an enema the evening before, and the morning of your appointment, they want your bowel area totally empty so the MRI gets a very clear picture of the entire area of the Prostate.
My prostate was somewhat enlarged and i was getting up a few times a night to urinate, I had a Rezum procedure in 2020 that was recommended by Prostate oncologist as the least invasive, and I’m very happy with that procedure.
Fast forward to 2024, my Prostate cancer MRI”s have not changed & last PSA was around 2.8.
I’m extremely grateful to DR. TROJANOVICH for all his support these past 5yrs and researching my situation and recommending a specialized prostate practice in California, I very well would’ve gone through an unnecessary prostatectomy surgery or radiation.
This has been a great education the past five years with the help of Dr. Trojanovich and one thing I have learned is that urologist and radiologist work hand-in-hand in this Prostate business and seldom recommend prostate MRIs (urologist and oncologist don’t get paid for MRIs )and imaging facilities have no protocols for prostate MRIs.
